Normally, sperm is produced in the testicles, stored in the epididymis (like a storage house), and then travels through the vas deferens before being released during ejaculation. However, sometimes the natural path for sperm release is blocked or doesn't function properly.

Common reasons for issues with sperm release include:

  • Blockages due to injury or infection
  • Congenital absence of the vas deferens (born without the tubes)
  • Non-obstructive azoospermia (very few sperm produced by the testicles)
  • Retrograde ejaculation (when sperm goes into the bladder instead of being ejaculated)

How Can Sperm Retrieval Help?

The good news? Sperm retrieval allows us to bypass these problems by directly collecting sperm from the testicles or epididymis. In most cases, even if there’s a blockage, we can successfully find sperm over 90% of the time. Even in cases where the testicles produce very few sperm, there’s still a 40% chance we can retrieve sperm. Once we collect the sperm, IVF/ICSI treatments can have success rates similar to those using naturally ejaculated sperm.

Different Ways to Retrieve Sperm

We use several sperm retrieval techniques, and we'll select the best one for your individual situation:

PESA (Percutaneous Epididymal Sperm Aspiration) - The Simple Approach

PESA involves using a very fine needle (like a tiny straw) to extract sperm directly from the epididymis where sperm are stored. It’s a quick and effective procedure.

TESE (Testicular Sperm Extraction) - The Direct Method

TESE is used when sperm is not found in the epididymis and must be taken directly from the testicle. A small incision is made in the skin to remove a tiny piece of testicular tissue, which is then examined to locate sperm-producing areas.

MESA (Microsurgical Epididymal Sperm Aspiration) - The Precise Method

MESA is similar to PESA, but we use a microscope to guide us, ensuring we collect sperm from the best areas. This method allows for a higher level of precision and higher quality sperm retrieval.

What to Expect During Sperm Retrieval

These procedures are all done on an outpatient basis, meaning you can go home the same day. Here’s what you can expect:

  • Anesthesia: We’ll use a local anesthetic to ensure comfort, and sometimes light sedation if needed.
  • Sperm Collection: Once we retrieve the sperm, we’ll examine it right away to ensure we’ve found what we need.
  • Freezing for Future Use: If sperm is collected successfully, it can be frozen and stored for use in future IVF cycles.

After the Procedure: Post-Retrieval Care

After your sperm retrieval procedure, you can usually return to work within 4-5 days. Here’s what to expect:

  • Supportive Underwear: Wear the special supportive underwear we provide for 48 hours to reduce swelling and discomfort.
  • Stitches: If any incisions were made, they will dissolve on their own within about 2 weeks, so no need for removal.
  • Mild Discomfort: You may experience some tenderness, mild pain, or slight cloudy discharge. In rare cases, a minor infection might occur, but we’ll prescribe medication to prevent this.
